# Form details

**Form Details** contains the form's name, internal description, **Create Event**, an optional event-template link, and **Return URL**. These settings determine how you recognize the form and whether a response starts an event.

The description helps you recognize the form in the Forms collection. It does not appear on the public page. The form link is public, so review the questions and sharing settings before you distribute it.

When **Create Event** is enabled, a submission starts a new event and links the response to that event. An optional event template supplies the starting event setup. Without a linked template, the event uses your [Event Settings](/reference/event-settings.md) defaults. The event date comes from the first **Event Date** field when the form has one. If no event date is supplied, FlowerBuddy uses a date 30 days after the submission date for an event created from that response.

**Return URL** is optional. When it is set, the submitted form can offer a **Return to Site** button. Leave it blank to omit that button.
